Custom shift knob I made for my truck. I used an EZ lok threaded insert with am M12 OD. The insert will angle into the larger diameter cutout toward the top of the shifter. Simply mirror the other half of the shifter and glue together. Since this was ASA, I used acetone to glue it together.
