Just another case for DPS5015. Assembling with screws 3,5x16, the board is fixed with M3x12 screws. DPS5015 Rocker Switch Banana Plug P.S. I do not know why, I made two inputs :) You can change the second input as an additional output. Or do not use it at all.

Update 05.06.2019 Redid the front panel (added XT60 plug and terminal connector) and rear panel (added XT60 plug). XT60 2 Way Terminal Connector

Update 17.08.2019 Added front panel without XT60 (only terminal connector and banana plug)
