This is a simple star shaped nameplate. The text is customizable in PrusaSlicer (have not used others, but would guess so). Just edit in the 3mf file when loaded.

Edges are chamfered, curved corners. Flat back.

120 mm x 120 mm x 4 mm (2 mm per color)

It's setup for a filament change at layer break. f the break gets lost, it is at 2.20 mm layer height.
