Two base pieces and one middle piece are needed for a single lead screw. An additional two M3 screws are required to hold one of the ends on the X-gantry bracket and the other with the lead screw nut.

Some slight sanding and silicone grease were required to ensure it slid smoothly in the X and Y directions but was tight enough to not have much slop in the Z direction. I printed it with Inland PLA+, but PETG might be better due to its slippery finish.
